1. Data Collection Practices

When contacting the practice and/or booking an appointment online, data is collected via cookies as well as information inputted via forms.

 

2. Purpose of Data Collection

We collect this data eg. name, email addresses, contact numbers, to make contact with the client/patient for the purpose of providing further information (directions, appointment reminders) as well as to populate and send statements following a treatment. These statements can be used to claim back from your Medical Aid provider should your plan cover Physiotherapy. 

The practice does from time to time send out information/marketing emails. One can unsubscribe from these at any time following an in mail link.

 

3. Consent

By accepting the cookie and privacy policy as well as contacting the practice, you consent to the use of provided information. As a new patient completing your in-depth patient details form, you will be required to consent to the use of data as well as consent to being treated by the physiotherapist. 

 

4. Sharing of Personal Information

Only in the case of an application for Disabled Golf, data (name, email address and assessment details of the applicant) is shared with EDGA (European Disabled Golf Association) when applying for an Access/WR4GD pass as stipulated in the consent form. This data is protected under secure servers and behind a firewall.

Outside of this, NO personal information is shared to other parties, unless referred to a specialist/X-ray department as requested/discussed in person with the Physiotherapist. At times one's Medical Aid/HPCSA can request information regarding the treatment. Should this occur, it will be brought to your attention and consent required (written, email) before any data is released.

 

5. Data Storage and Security

The practice website is hosted on a Wix.com platform. Wix.com provides us with an online platform that allows you to contact us via a contact form. Your data may be stored through Wix.com's data storage, databases and the general Wix.com's applications. This is stored on secure servers behind a firewall.

When booking an appointment online, data is encrypted and stored on secure servers and protected behind a firewall. Data is encrypted in transit (between the practice, clients and the servers).​ Medical Data records are stored up to 7 years following one's last appointment and/or up to the age of 21 years in minors.

 

6. User Rights

If you would no longer want the practice to process your data or would like to correct inaccurate information, please contact us on nikkiparkerphysio@gmail.com. Please note due to South African Medical Law, health records are stored up to 7 years following one's last appointment and/or up to the age of 21 years in minors.

 

7. Updates to the Privacy Policy

We reserve the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take immediate effect upon their posting on the website. If we make material changes to this policy, we will notify you here that it has been updated, so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we use and/or disclose it.
